REGISTRATION CHECKLIST
Use this checklist to help gather the information that you need to register for
MTEL paper-based testing.

Select a registration method—Internet, U.S. mail, or telephone.
Depending on the registration period (regular, late, or emergency),
you have the following choices:
Candidates who registered for the MTEL previously may also register by telephone
during the regular or late registration period. See the telephone reregistration
schedule in "Test Dates."

| 2. Gather the personal information you need to register. |
| When you create an account or register for the MTEL, you will be asked to provide the following information: |
- Name
- Address
- Daytime telephone number (required for Internet registration)
- Date of birth (required to access your test results online)
- Social security number or Department-issued identification number*
- E-mail address (required for Internet registration)
- First (native) language
- Ethnicity (optional)
- Gender (optional)
- License category (refer to "Licensure Information")
- Specific license sought

* If you do not have a social security number, request an identification number from the
Department of Elementary and Secondary Education by sending an e-mail to mrodriguez@doe.mass.edu.

| 3. Review the background questions. |
|
If you are registering for the MTEL and your license category is Academic (PreK–12),
you will be asked to answer some background questions.
Refer to "Educational
Background Questions" to prepare answers to
the questions you will be asked during registration.

| 4. Review the Rules of Test Participation. |
|
Before you register,
you must read and agree to abide by the policies
stated in "Compliance
with Testing Rules," including the Rules of
Test Participation, and all procedures and policies
contained in the 2011–2012 MTEL Registration Bulletin.
