BEFORE YOU REGISTER
EDUCATIONAL BACKGROUND QUESTIONS

If your license category is “Academic (PreK–12),” you must provide ALL required information during registration, including responses to the following questions, or your registration materials will be returned to you unprocessed and your test date may be delayed as a result.

If your license category is either “Vocational Technical” or “Adult Basic Education,” you are not required to answer these questions.

Providing accurate and complete background information about yourself is important. All of the information that you provide will be kept confidential.

If you register on the Internet, you will answer the following questions interactively as you register. If you register by mail, enter your responses to the following questions on the registration form. Each question number corresponds to the section of the registration form where you will record your response.

14. Education and Institution Information (required)

A. What is the highest educational level you have attained to date? (Select one option only.)

1. Freshman (first year of college/university)
2. Sophomore (second year)
3. Junior (third year)
4. Senior (fourth or final year)
5. Earned bachelor’s degree
6. Earned master’s degree in education
7. Earned master’s degree in a subject area
8. Earned other advanced degree
9. I have not attended or applied to an institution of higher education.
(If your response is “9,” skip to “Educator Preparation.”)

B. Indicate the college or university that granted or will grant you a bachelor’s degree.
  • If you received or will receive your bachelor’s degree from a Massachusetts institution, enter the two-digit code from “Institution Codes.”

  • If you received or will receive your bachelor’s degree from a Massachusetts institution that is not listed, enter “99” and write the name of your institution in the space provided.

  • If you received or will receive your bachelor’s degree from an institution outside the state of Massachusetts, enter “90.”

C. If you selected “6,” “7,” or “8” in “Education and Institution Information,” identify the college or university that granted you the advanced degree(s).
  • If you received your advanced degree(s) from a Massachusetts institution, enter the two-digit code from “Institution Codes” for each institution (up to two).

  • If you received your advanced degree(s) from a Massachusetts institution that is not listed, enter “99” for each institution (up to two) and enter the name(s) of the institution(s) in the space provided.

  • If you received your advanced degree(s) from an institution outside the state of Massachusetts, enter “90.”

15. Undergraduate Primary Academic Major (required)

Enter the two-digit code(s), found in the table below, for the discipline(s) constituting your undergraduate primary academic major(s) (up to three).

17. Educator Preparation (required)

Indicate where your educator preparation for the license you are seeking will take place, is taking place, or took place.
  • In-state (proceed to “In-State Code”)

  • Out-of-state (skip to “Out-of-State Code”)

  • Not applicable (skip to “Additional Colleges or Universities to Receive Results”)
18. In-State Code (required, if your response to “Educator Preparation” is “In-state”)

Complete this section if your educator preparation will take place, is taking place, or took place at a Massachusetts institution.

A. Indicate your Massachusetts educator preparation program status. (Select one option only.)

1. Applying to a Massachusetts educator preparation program
2. Enrolled in a Massachusetts educator preparation program
3. Completed a Massachusetts educator preparation program
B. Identify your educator preparation program by entering the two-digit code from “Institution Codes.” This program will automatically receive your test results.
C. Indicate whether or not you authorize, in addition to your test results, your social security number to be sent to this educator preparation program.
19. Out-of-State Code

Complete this section if your educator preparation for the license you are seeking took place outside the state of Massachusetts.

Enter the postal code that indicates the state in which you completed educator preparation requirements.
20. Additional Colleges or Universities to Receive Results (optional)

You may indicate up to three additional Massachusetts colleges or universities to receive your pass/fail status by entering the two-digit codes from “Institution Codes.” The Massachusetts institution you indicated in “In-State Code” will automatically receive your test results.

If you need a copy of your score report to be sent to your out-of-state educator preparation program institution, or if you coded “99” in Section 14B, indicating you received or will receive a bachelor’s degree from a Massachusetts institution that is not listed in “Institution Codes,” submit a letter that states your request. If you are mailing your registration form, enclose the letter with the registration form; if you are registering online, mailing instructions will be provided.

Be sure to include the name, title, and institution address of the individual to whom the report must be sent.
